29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about May 3, 2017, and at times prior thereto, the employer did not provide machine guards to the following equipment, exposing the employees to the hazards of being caught in rotating parts and/or moving parts:  a. In the First Stage Area on the Short Wool Processor, the guard door was chained opened over the conveyor belt. b. In the Scouring Line Area on the Scouring Line, the entire length of the line had exposed conveyors belts, and agitating fingers.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed.  On or about May 3, 2017, and at times prior thereto, the sprocket and chain assembly for the water press roller was not enclosed, exposing employees to the being caught in the rotating sprocket and chain.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that the strain relief prevents pull from being directly transmitted to joints or terminal screws.  On or about May 3, 2017, and at times prior thereto, the Dayton metal floor fan had the electrical power cord pulled from the fan casing, exposing employees to the hazard of electrical shock.
