29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section (fall protection training) by preparing a written certification record which contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: a) 1520 Toronita Street, York PA - The employer did not prepare a written certification verifying that employees had been trained in hazards related to falls from elevation, on or about April 28, 2017. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) 1520 Toronita Street, York PA - On or about April 28, 2017, employees were not provided a proper ladder or stairway to access and egress the roof of the corridor and were exposed to an approximate 9 foot - 10 foot fall to the floor below. Employees used 8 foot portable step ladders and hoisted themselves onto the surface. No abatement certification or documentation required.
