29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Jobsite: On, or about, 02/27/2017 employees performing construction activities were not protected by a guardrail system, net system, or personal fall protection exposing the employees to a fall of approximately 26 feet in the following instances:        (a)  Building 5-7: Employees were relocating roofing material from the power ladder to the far end of the building without the use of fall protection.        (b)  Building 9-11: Employees were installing roof sheathing and barrier paper without the use of fall protection.         (c)  Building 15-17: An employee was installing a ridge cap without the use of fall protection.

(b) Building 15-17 - the extension ladder did not extend at least three feet beyond upper landing surface whereby the ladder was not secured at the top and a grasping device was not installed.
