29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about May 4, 2017, at the above addressed worksite, an employee was exposed to a fall of approximately 14 feet 10 inches when performing masonry work from the basket of an aerial lift while not wearing a harness with a lanyard attached to the boom or basket of that lift.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about May 4, 2017, at the above addressed worksite, an employee was exposed to a fall hazard of approximately 16 feet when performing masonry work on a walking/working surface while not being protected from falling to the lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about May 4, 2017, at the above addressed site, an employee was exposed to a fall hazard when performing masonry work on a walking/working surface that was not provided with training that would enable him to recognize the hazards of falling.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  On or about May 4, 2017, at the above addressed site, an employee exposed to fall hazard of approximately 16 feet when using an aluminum extension ladder section that could not extended 3 feet past the upper landing surface, it was not secured to prevent accidental displacement and a grasping device was not provided.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about May 4, 2017, at the above addressed site, an employee gaining access to and from a roof was exposed to a fall of approximately 16 feet when using one section of an extension ladder that could not reach the upper landing surface and was not using it for the purpose which it was designed for.
