OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    The employer is failing to protect employees from struck-by hazards while using a company manufactured spreader beam.  This most recently occurred on May 4, 2017 at 13433 Centech Road, Omaha, Nebraska, where employees were using an unmarked and unrated spreader beam to lift and hoist steel materials weighing up to 15,000 pounds.      Among other methods, one feasible and acceptable means of abatement would be to replace the spreader beam lifting device with one that is marked and rated in accordance with ASME B30.20-2013, Below-the-Hook Lifting Device, Chapter 20-1, page 13.

The most recent occurrence was evident during the inspection conducted on May 4, 2017 at the facility located at 13433 Centech Rd, Omaha, NE 68138.  The employer has employees lifting loads greater than the maximum allowable weight annotated on the crane.

The most recent occurrence was evident during the inspection conducted on May 4, 2017 at the facility located at 13433 Centech Rd, Omaha, NE 68138.  Overhead cranes used to transfer and move various sizes, weights, and shapes of steel were not properly marked with the rated load on each side.

29 CFR 1910.1200(f)(6): The employer used written materials, such as signs, placards, process sheets, or batch tickets in lieu of affixing labels to individual stationary process containers that failed to identify the container(s) to which they were applicable and did not convey the information required by 29 CFR 1910.1200(f)(5):    The employer is failing to protect employees from the hazards associated with the chemicals used in the workplace.  The most recent occurrence was evident during the inspection conducted on May 4, 2017 at the facility located at 13433 Centech Rd, Omaha, NE 68138.  The employer is exposing employees to solvents which have been transferred from their original container to a secondary container that has not been properly labeled.  The containers are in the form of hand-held spray bottles and are filled by employees on one shift and left on the workbench for use by other employees on the same or a different shift.

Powered industrial vehicles which are used to move materials around the facility did not have a data plate with weight capacity limitations identified.
