29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:      a) On or about May 4, 2017 and at times prior, two employees were exposed to approximately 20 feet fall hazard while working inside the basket of a boom lift. The employees did not have their personal fall arrest system attached to the basket of the boom lift.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      a) On or about May 4, 2017 and at times prior, employees were exposed to approximately 10 feet fall hazard while working on the metal roof of a commercial building. The employees were not protected with a fall protection system.

The employee was delivering a piece of metal to the roof while riding on the fork of the forklift.
