On or about 5/02/2017, employees from Eloy Zaro were installing shingles on a roof of a residential building located at 101 Denning Drive in Wentzville, MO. Two employees were not wearing eye protection while using pneumatic nail guns.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On or about 5/02/2017, employees from Eloy Zaro were installing shingles on a roof of a residential building located at 101 Denning Drive in Wentzville, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The owner and four employees were observed on the roof of the building approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to the upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 5/02/2017, employees from Eloy Zaro were installing shingles on a roof of a residential building located at 101 Denning Drive in Wentzville, MO. The ladders the employees were using to access the roof were not extended three feet beyond the upper landing surface or edge of the roof.
