29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On  June 9, 2017, Mace "Bud" Cleghorn, dba Morton Construction, Inc., was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:        a)  Citation 1 Item 1a & b, and     b)  Citation 1 Item 2.   Mace "Bud" Cleghorn, dba Morton Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 1168082, citation number 2, item number 2 and was affirmed as a final order on 03/13/2017, with respect to a workplace located at 1227 W. Barker, East Peoria, IL.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about May 5, 2017, Bud Cleghorn dba Morton Construction, Inc. had employees exposed to fall hazards on a residential construction site in Morton, Illinois.  The employer did not have an effective safety and health program with specific work rules, competent person oversight, and employee training to govern employees activities.  Bud Cleghorn dba Morton Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 1168082, citation number 1, item number 1a and was affirmed as a final order on December 20, 2016, with respect to a workplace located at 1227 W. Barker Avenue, Peoria, IL 61606.

The employer did not have a competent person conducting frequent and periodic safety inspections at the construction site.         Bud Cleghorn dba Morton Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.20(b)(2), which was contained in OSHA inspection number: 1168082, citation number 1, item number 1b and was affirmed as a final order on December 20, 2016, with respect to a workplace located at 1227 W. Barker Avenue, Peoria, IL 61606.

Bud Cleghorn dba Morton Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13) which was contained in OSHA inspection number: 1168082, citation number 1, item number 4a and was affirmed as a final order on December 20, 2016, with respect to a workplace located at 1227 W. Barker Avenue, Peoria, IL 61606.
