29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about, May 5, 2017, at the above addressed jobsite, employees conducting masonry operations from a tubular welded framed scaffold, fourteen (14) feet four (4) inches above the ground, were exposed to fall hazards due to the lack of a guardrail system and end-rail system. Pignato Masonry, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(i) which was contained in OSHA inspection number 1165774 citation number 1 item number 1 and was affirmed as a final order on September 27, 2016 with respect to a workplace located at 19180 LaGrange Road, Mokena, IL.
