29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a)  Kiln #1, on or about 5/5/17:  Employees accessing the top of the kiln for preventive maintenance purposes and inspection of counter weight cables were not provided with fall protection.  Employees were exposed to falling approximately 15 feet from the unprotected sides.    b)  Kiln #2, on or about 5/5/17:  Employees accessing the top of the kiln for preventive maintenance purposes and inspection of counter weight cables were not provided with fall protection.  Employees were exposed to falling approximately 15 feet from the unprotected sides.    c)  Block Machine, on or about 5/5/17:  Employees who perform daily cleaning activities while standing on an open sided platform above the hopper are exposed to falling approximately 8 feet 4 inches down into the hopper.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:    a)  Paver Building, on or about :  480 volt control panel serving the Paver Machine was not provided with guarding to prevent physical damage from forklifts being operated in the immediate area.
