29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).   At the establishment, in the shipping department, employees operated a Case 588G, 16,000 pound forklift, serial number 64173512812, model number 8WMVR,  and were not trained by the employer in the safe operation of the forklift, exposing employees to the hazard of being struck by the lift.
