Coblentz Custom Sawing - On or about 5/4/17, and at times prior thereto, safety glasses were not worn by employees while performing work activities that exposed them to eye injury hazards.  The employees perform work activities that included, but were not limited to, operating saws.

Coblentz Custom Sawing - On or about 5/4/17, and at times prior thereto, the company did not have elements of a lockout/tagout program that included, but was not limited to, lockout/tagout procedures, employee training, lockout/tagout hardware or periodic inspections.  Employees perform servicing and maintenance on machines that include, but are not limited to, chop saws, head saws, and conveyors.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:        A.)  Coblentz Custom Sawing - On or about 4/18/17, and at times prior thereto, a chop saw was not properly guarded.  The saw was not guarded with a retractable guard.  The guard did not cover the entire blade when material was not being cut but the saw was still running.
