29 CFR 1926.552(a)(1): The employer did not comply with manufacturer's specifications and limitations applicable to the operation of all hoists and elevators: Jobsite located at 150 White Rock Drive, Windsor, Connecticut: On, or about, May 4, 2017, employees were observed climbing down the track of a RCG platform hoist instead of using a separate ladder. The platform hoist instructions clearly stated that a ladder was required for accessing upper work levels and that climbing on the track was unsafe.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: Jobsite located at 150 White Rock Road, Windsor, Connecticut: On, or about, May 4, 2017, employees were installing shingles on a roof without any fall protection system in use. AMPM Home Improvements, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 898353, Citation Number 1, Item Number 2 and was affirmed as a Final Order on April 24, 2013, with respect to a workplace located at New London, Connecticut.
