29 CFR 1910.25(b)(1): The employer did not ensure handrails, stair rail systems, and guardrail systems were provided in accordance with 29 CFR 1910.28:    a) Stairs at the door way into the shipping dock area on the north side of facility:    On or about 5-2-17 the stairs were not provided with stair rail systems with hand rails on open sides.    b) Exit door stairs at southeast corner of facility:            1) On or about 5-2-17 the stairs were not provided with a stair rail system with hand rail on the open side.      2) On or about 5-2-17 the stairway landing was not provided with a standard guard rail system.
