29 CFR 1926.453(b)(2)(v): A body belt or harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:      a)  Southwest side of the Commercial Building - On or about May 8, 2017, an employee installing metal framing spacers from an aerial lift did not have his harness lanyard attached to the aerial lift basket, exposing himself to a fall hazard of 13 feet.      No abatement certification or documentation required.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold, or work platform, trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    a)  Southwest side of the Commercial Building - On or about May 8, 2017, the employer had not instructed each employee working from the JLG aerial lifts to recognize the hazards associated with the type of mobile work platform being used and to understand the procedures to control or minimize those hazards, including but not limited to the nature of any electrical hazards, fall hazards and falling object hazards in the work area; the correct procedures for dealing with electrical hazards; the correct procedures for maintaining and disassembling the fall protection systems being used; and the maximum intended load and the load-carrying capacities of the aerial lift being used.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
