29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  a) On May 8, 2017, an employee performing masonry work used a 20 pound propane cylinder to fuel a torch without a fire extinguisher present.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On May 8, 2017, employees were performing residential roofing work from the second floor balcony without fall protection, exposing the employees to fall hazards greater than six (6) feet.  b) On May 8, 2017, employees performing residential roofing work on the second floor of a building under construction were exposed to fall hazards from the second floor to grade when working near unprotected wall openings on the second floor.  c) On May 8, 2017, employees performing residential roofing work on the second floor of a building under construction were exposed to fall hazards when working near a floor hole around the stairway opening between the first and second floors which was missing a midrail.
