29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: North roof area: On or about May 3, 2017, and times prior to, employees engaged in preparing a roof for re-roofing used an electric circular hand saw to remove bad roof decking. The saw was powered from cord sets supplied by an interior building receptacle with no ground fault protection available. This exposed the employees to the hazard of electrical shock.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Roof north side: On or about May 3, 2017, and times prior to, employees engaged in the preparing the roof for re-roofing were not using available personal fall protection equipment or any of the other allowable fall protection systems. This exposed the employees to the hazard of falls over 6 feet to the ground below.
