29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) Roof - Employees performing roofing activities were exposed to a fall hazard of approximately thirty (30) feet on an eight by twelve (8 X 12) pitch roof without the protection of a guardrail system, safety net system, personal fall arrest system or an alternate fall protection measure, on or about May 8th, 2017.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Roof Access - Employees using an extension ladder to access and egress from the roof were exposed to an approximate thirty (30) foot fall hazard from the ladder rails not extending at least three (3) feet above the working surface, on or about May 08, 2017.    b) Roof Access - Employees using an extension ladder to access and egress from the roof were exposed to an approximate eight (8) foot fall hazard from the ladder rails not extending at least three (3) feet above the working surface, on or about May 08, 2017.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
