29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Garage Roof - Employees performing roofing activities were exposed to a fall hazard of approximately ten (10) feet without the protection of a guardrail system, safety net system, personal fall arrest systems or an alternate fall protection measure, on or about May 8th, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Garage Roof Access - Employees using an eight (8) foot step ladder to access and egress from the roof were exposed to a fall hazard of approximately seven (7) foot four (4) inches to the ground without the ladder rails extending at least three (3) feet above the working surface, on or about May 8th, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
