29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a). Worksite located at 301 W. Armour Blvd., Kansas City, Missouri- The employer failed to inspect equipment prior to use and thusly exposed employees to shock or electrocution hazards in that an extension cord being used had its' outer insulation breached, exposing the inner wires, and the radial saw being used had its' grounding pin removed.
