29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck (PIT):    Warehouse: The employer is exposing employees to unsafe conditions.  The employees are exposed to struck-by hazards where untrained employees are permitted to operate powered industrial truck.     On and about 05/09/2017 an untrained employee operated a Crown Wave 2 PIT (Model Number WAV 50-118, Serial Number 9A146780) exposing employees to struck-by hazards.

29 CFR 1910.178(q)(7):  Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected:   Warehouse: (A) Employer is exposing employees to hazardous conditions.  Employees are operating powered industrial equipment daily without conducting required safety examinations prior to operating the equipment.   On and about 05/02/2017 employees operated a powered industrial truck, Crown Wave 2 PIT (Model Number WAV 50-118, Serial Number 9A146780) without having performed the required daily safety examination prior to operating it.

29 CFR 1910.303(b)(7)(iv): The employer did not remove from service damaged electrical equipment with defects such as, but not limited to, broken parts, bent parts, parts cut or deteriorated by corrosion, chemical action, or overheating, which may adversely affect safe operation or mechanical strength of electric equipment:    Cleaning area: Employer is exposing employees to hazardous conditions.  The employer allows employees to use damaged electrical equipment exposing the employees to electrical hazards.  On and about 05/09/2017 a damaged electrical duplex receptacle was used by employees.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:   Warehouse exit door:  Employer is exposing employees to hazardous conditions.  The employees are exposed to electrical hazards where there is no easy access to electric equipment.   On and about 05/09/2017 a pallet of products was left on workplace floor blocking access to an electric panel box.
