29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    a) Worksite was located at 201 Angelo Cifelli Drive, Harrison, NJ 07029    Employees were exposed to electrical hazards as a Rigid Spade Handle Mud Mixer was being used with a bad cord that had bare conductors exposed.    Violation was observed on or about May 8th, 2017.

29 CFR 1926.1053(a)(1)(i): Self supporting portable ladders were not capable of supporting, without failure, at least four times the maximum intended load:    a) Worksite was located at 201 Angelo Cifelli Drive, Harrison, NJ 07029    An employee was exposed to fall hazards as he was using a grout bucket as a makeshift portable ladder that was not designed to support the intended load.    Violation occurred on or about May 8th, 2017.
