29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) In the loading dock area, on or about May 5, 2017 and at times prior to, the cardboard bailer was cycled without the manufacturer installed guard gate closed, exposing employees to an amputation/crushing hazard.

29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, was not provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employee: (a) Throughout the facility, on or about December 6, 2016 and at times prior to, the employer did not provide basic advisory information contained in Appendix D to employees who used filtering face piece respirators on a voluntary basis, resulting in a respiratory and dermal hazard.
