D & M Tool, Inc. - On or about 5/9/17, and at times prior thereto, the Roll-in vertical band saw did not have an adjustable guard to safeguard the unused portion of the blade and the blade guide rolls. B.) D & M Tool, Inc. - On or about 5/9/17, and at times prior thereto, the Roll-in band saw did not have guards covering the area at the bottom of the saw housing.
