29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      On or about 5/10/2017, at the bakery area of the above addressed jobsite, the employer did not establish an energy control program, resulting in the exposure of employees to a caught by hazard while performing servicing activities such as, but not limited to;  cleaning of a mixer, adjustment of depositor's knives; changing of depositor's die, etc.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:     On or about 5/10/2017, at the bakery area of the above addressed worksite, employees were exposed to an electrical hazard while working in the proximity of a conveyor (i.e. Avatar, Serial #700469), which had its equipment grounding conductor disconnected.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:   a. On or about 5/10/2017, at the bakery area of the above addressed jobsite, an electrical cord lacked of strain relief where entering a bag filler machine, exposing employees to an electrical hazard.  b. On or about 5/10/2017, at the bakery area of the above addressed jobsite, an electrical cord lacked of strain relief where entering the attachment plug of a depositor, exposing employees to an electrical hazard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about 5/10/2017, at the above addressed jobsite,  employees were exposed to a chemical hazard due to lack of information and training, as evidenced by, among other conditions; employees lacked knowledge of the location and availability of the required safety data sheets and the employer's hazard communication program; employees lacked knowledge of the hazards of presented by cleaning products used for sanitation purposes, etc.
