29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) In the meeting breakroom area, a Belkin Relocatable Power Tap A, used to power a TV and radio chargers was  permanently plugged into an extension cord A, in violation of listing and labeling requirements.    (b) In the meeting break room extension cord A was permanently plugged into Relocatable Power Tab B in violation of listing and labeling requirements.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle supplied a total cord- and plug-connected load in excess of the maximum specified in Table S-4:    In the meeting room breakroom a relocatable power tap rated for 15 amps and a maximum load of 12 amps was supplying current in excess of 12 AMPs to electrical devices such as but not limited to a Sears Refrigerator Model 9738682, Serial ED3776882 drawing 6.5 AMPs, an Emerson Microwave Model MWS 119SB drawing 12.5 AMPS, a television and several portable radio chargers.
