29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: 6787 Bear Creek Drive, Saint Louis, MO 63129, on or about 5/10/2017, workers were observed using a Werner Equalizer, Model: D1824-2EQ, Type 1, 250lb capacity aluminum extension ladder, and the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed.  The workers were exposed to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 6787 Bear Creek Drive, Saint Louis, MO 63129, on or about 5/10/2017, workers were observed on top of an 4:12 pitch roof, approximately 10 feet 2 inches eave to ground level, actively installed new roof shingles and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.  JJ RESTORATION, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1099830, citation number 1, item number 2, issued on 11/10/2015 and was affirmed as a final order on 12/9/2015, with respect to a workplace located at 11 Blackwood Lane, Saint Peters, MO 63376.  JJ RESTORATION, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 923021, citation 1, item number 2, issued on 11/17/2013, and was affirmed as a final order on 12/2/2013, with respect to a workplace located at 2504 Lucas and Hunt Road, Saint Louis, MO 63121.
