29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   a) Plymouth Flea Market, SW side of Bldg: Three employees were not protected from falling while installing metal flashing on top of a low-slope commercial roof, exposing employees to an approximate twenty-five (25) ft. fall, observed on or about May 10, 2017. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Plymouth Flea Market:  A 30-foot portable extension ladder was used for access to a roof at least 25 feet high from ground level and the ladder side rails did not extend at least 36 inches nor were the side rails tied off onto a sturdy structure. Employees were exposed to fall hazards from a ladder on or about May 10, 2017.   b) Plymouth Flea Market:  A 12-foot portable extension ladder was used for access to a roof at least 25 feet high from ground level and the ladder side rails did not extend at least 36 inches nor were the side rails tied off onto a sturdy structure. Employees were exposed to fall hazards from a ladder on or about May 10, 2017.
