29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    On or about May 11, 2017, an employee performing residential framing operations was exposed to potential eye or face injury while operating a pneumatic nail gun, the employer failed to ensure the exposed employee wore eye protection.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 11, 2017, an employee was not protected from a fall while performing residential framing activities, exposing the employee to a fall of 19 feet.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
