29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: On or about 5/10/2017, at the above addressed work site, an employee was exposed to a fall hazard when the employer allowed the employee to attach his safety rope to a vent pipe while applying cement for the roof tiles.
