(b) On or about May 11, 2017 and at times prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Turn-Pro mill, Model No. GS15V, Serial No. 141170T was operated in the project department located at 675 12th Avenue NE, West Fargo, North Dakota. (c) On or about May 11, 2017 and at times prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Turn-Pro mill, Model No. GS15V, Serial No. 961291Y was operated in the project department located at 675 12th Avenue NE, West Fargo, North Dakota.
