On or about 4/25/2017 at 376 Douglas Rd. East in Oldsmar, Florida, an employee suffered a work related injury and was hospitalized for surgery. The employer was aware of the in-patient hospitalization on 4/25/2017 around 5:00pm. The employer notified OSHA on 5/5/2017 at 4:00pm of the in-patient hospitalization.
