On or about May 4, 2017, at the fabrication shop, the Caterpillar T40B forklift being used at the site to move equipment and materials was defective and unsafe in that the backup alarm was not operational, and the horn was not working, exposing employees to struck-by hazards. b. On or about May 4, 2017, at the fabrication shop, the Sellick SD100 forklift being used at the site to move heavier equipment and large pieces was defective and unsafe in that the backup alarm was not operational, and the horn was not working, exposing employees to struck-by hazards.
