29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  On 6-15-17  a) An employee was exposed to an 8-hour time weighted average noise level of  87.8 dBA(equivalently a dose of 74.1%)in excess of the 85dBA (equivalently a dose of 50%), while using power tools to construct wooden containers and pallets.    b) An employee was exposed to an 8-hour time weighted average noise level of  88.7 dBA (equivalently a dose of 83.7 %)in excess of the 85 dBA (equivalently a dose of 50%) action level, while using power tools to construct wooden containers and pallets.    c) An employee was exposed to an 8-hour time weighted average noise level of  88.9dBA (equivalently a  dose of 85.8%) in excess of the 85 dBA (equivalently a dose of 50%) while building while using power tools to construct wooden containers and pallets.    d) An employee was exposed to an 8-hour time weighted average noise level of 91.5 dBA (equivalently a dose of 122.8%) in excess of the 85dBA (equivalently a dose of 50%) while using power tools to construct wooden containers and pallets.    A hearing conservation program was not fully implemented including noise monitoring, baseline and annual audiometric testing, training, and the posting of the hearing conservation standard 1910.95.
