29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment: (a) Project Renewal, 651 West 168th St. New York, NY 10032. Employees working with hazardous chemicals, such as, but not limited to, Sakrete Sand Mix which contains Quartz. Skin contact with this product can cause dermatitis and chemical burns. The employer did not asses the workplace hazards to determine the need for personal protective equipment. The condition was noted at or around May 11, 2017. (b) Project Renewal, 651 West 168th St. NY, NY 10032. Project Renewal employees were exposed to asbestos (Chrysotile 12% tiles) hazard during renovation project. The employer did not asses the worksite to determine if the hazards of asbestos containing material (ACM) or presume asbestos material (PACM) existed before initiating renovations work in a building built prior to 1980. The condition was noted at or around May 11, 2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.

Employer(s) and building and facility owner(s) did not exercise due diligence in complying with these requirements to inform employer(s) and employee(s) about the presence and location of ACM and PACM.
