29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: 1) Retail Store, 3805 Dallas-Highway, Marietta, GA: On or about May 11, 2017, the employer exposed employees to fall hazards of approximately 20 feet, in that, fall protection was not utilized while installing metal panels on the roof. 2) Retail Store, 3805 Dallas-Highway, Marietta, GA: On or about May 11, 2017, the employer exposed employees to fall hazards, in that fall protection was not utilized while working from a JLG aerial lift (model 400S, serial 0300071154) to access and egress the top level on the roof.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. 1) Retail Store, 3805 Dallas Highway, Suite 680, Marietta, GA: On or about May 11, 2017, the employer exposed employees to fall hazards of approximately 30 feet, in that fall protection was not utilized while installing metal panels on the roof. The Marietta Welding, LLC was previously cited for a violation of this occupational safety and health standard,29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 1102776, citation number 01, item number 001 and was affirmed as a final order December 22, 2015, with respect to a workplace located at 540 Jimmy Deloach Parkway, Savannah, GA 31407.
