29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water:  (a) At the barge terminal, on the first tier of the barge tower, an elevated concrete pillar, there was no guardrail between the edge of the stairway platform rail and the steel crossbeam of the tower structure.  The opening measured 58 inches high, 22 inches wide at the top of the rail and 39 inches wide at the base of the platform and exposed employees to falls of approximately 9 feet to a concrete pad.  (b) At the barge terminal, on the first tier of the barge tower, an elevated concrete pillar, there was no fall protection at the opening of top of the ladder used to access the barge where falls into the water was a hazard when the barge was not being worked.  The elevated concrete pillar was at least 9 feet above the water level.
