29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  A.  On or about May 17, 2017, Warehouse:  Directional signs were not provided to identify the direction of travel to the nearest emergency exit for employees working within product aisles.

29 CFR 1910.303(b)(7)(iv): Electric equipment had damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:   A.  On or about May 17, 2017, near bay 45:  The GE A Series 120/208V, 225 amp panel board showed evidence of damage.  Screws were missing from the dead front, which allowed it to shift, leaving a quarter inch gap on the right of the 20 amp breakers.  Two blanks beneath the breakers on the right side of the panel were loose.  The openings could allow the entry of foreign objects such as screws or dust into the panel, which could cause it to overheat.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   A.  On or about May 17, 2017 near Bay 45:  The working space in front of the 120/208V, 225 amp GE A Series electrical panel was blocked by the placement of a pallet of product wrapped in black plastic.  The panel provided power to battery charging units and other equipment in the warehouse.
