29 CFR 1910.22(d)(2): Hazardous conditions on walking working surfaces were not corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired  (a) Date of Violation: 05-08-2017; Location: Salem VA Medical Center, 1970 Roanoke Blvd. Salem, VA 24153, Condition: The employer did not ensure the attic access concrete steps were corrected or repaired before employees were required to use them.  The concrete step surfaces contained numerous areas with missing, cracked, and chipped sections of concrete and were in disrepair.  Riser height and tread depth were at irregular and inconsistent depths and heights.  Instance #1:  Building #09 Attic Access - The concrete step surfaces contained numerous areas with missing, cracked, and chipped sections of concrete and were in disrepair.  Riser height and tread depth were at irregular and inconsistent depths and heights.  Instance #2:  Building #11Attic Access - The concrete step surfaces contained numerous areas with missing, cracked, and chipped sections of concrete and were in disrepair.  Riser height and tread depth were at irregular and inconsistent depths and heights.  Instance #3:  Building #73 Attic Access - The concrete step surfaces contained numerous areas with missing, cracked, and chipped sections of concrete and were in disrepair. Instance #4:  Building #74 Attic Access - The concrete step surfaces contained numerous areas with missing, cracked, and chipped sections of concrete and were in disrepair. Instance #5:  Building #76 Attic Access - The concrete step surfaces contained numerous areas with missing, cracked, and chipped sections of concrete and were in disrepair. 29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "EXIT":  (a) Date of Violation: 05-08-2017; Location: Salem VA Medical Center, 1970 Roanoke Blvd. Salem, VA 24153, Building #14 Condition: The employer did not ensure that doors designated as emergency exits were marked with "EXIT" signs. The south exit double doors discharging to the loading dock area was not marked as an exit.  The doors contained an electromagnetic locking mechanism and the doors did not swing in the direction of egress travel.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

The electrical cord enclosure fitting and locking nut was not installed where it passed through the sharp metal hole where it entered into the equipment.  The cord was for the charging system located onboard a Werres Corporation E-Z-GO Brand scooter.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
