Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) 134 Innis Ave, Poughkeepsie NY (Mountain brook Apartments): On or about 5/10/2017, Employees engaged in roofing work was not protected from falling by a fall protection system, exposing them to falls greater than 6 feet.
