29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: (a) Associated Voice DBA Front Steps at 400 South Colorado Blvd, Suite 790, Denver CO, 80246: On or before 05/12/17, the employer did not ensure that passageways and walkways were kept clear of tripping hazards. The walkways in the development and product team areas included electrical equipment, electrical wires, and power strips that presented tripping hazards. This condition exposed employees to the tripping hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) AssociatedVoice LLC DBA Front Steps at 400 South Colorado Blvd, Suite 790, Denver CO, 80246: On or before 05/12/17, the employer did not ensure that electrical equipment was used and installed in accordance with the manufacturer's instructions. The employer allowed the use of relocatable power taps that were connected in series (daisy chained) for use in providing power to multiple electrical devices. This condition resulted in electrical shock and fire hazards.
