29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) Navy Gateway Inn and Suites: On or about 05/11/2017, the ground fault circuit interrupter receptacle used by construction site employees was tested and found to have reversed polarity as a result employees were exposed to a possible electrical shock hazard. No abatement certification or documentation required.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) Navy Gateway Inn and Suites: On or about 05/11/2017, the wire nut connections that supported the light socket conductor and Type NM cable wires were not provided with strain relief. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
