29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Deschamps Mat Systems Inc. at 218 Little Falls Road, Cedar Grove, NJ  The employer did not perform a hazard assessment to identify potential hazards to workers repairing mats by melting fibers of recycled polyethylene.  Violation occurred on or about 5/19/17.  Express Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1116013, citation number 1, item number 1 and was affirmed as a final order on 5/17/16, with respect to a workplace located at Wei-Chuan USA Inc., 80 Amity St., Jersey City, NJ.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a)  Deschamps Mat Systems Inc. at 218 Little Falls Road, Cedar Grove, NJ  The employer did not implement a written hazard communication program for workers repairing edges of mats made from recycled polyester fibers.  The repairs were made by melting the polyester (polyethylene terphthalate), exposing workers to hazardous substances including but not limited to acetaldehyde and carbon monoxide.  Workers did not receive general hazard communication training.  Violation occurred on or about 5/9/17.
