29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) At the commercial construction site located at at1500 W College St, Springfield, MO 65806:  Employees were exposed to fall hazards while working on a roof without the use of fall protection. This was demonstrated on 5/10/2017 when two employees were allowed to work from an approximate distance of 23 feet on the highest point to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) The employer failed to provided safe access to employees at a commercial construction site located at1500 W College St, Springfield, MO 65806, when employees were using portable ladders to gain access to a roof. This was demonstrated on 5/10/2017 when a ladder was being used to gain access to the roof and the ladder was not placed at least 3 feet above the upper landing.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    a) The employer failed to provided safe access to employees at a commercial construction site located at1500 W College St, Springfield, MO 65806, when employees were using a makeshift stairway comprised of cinder blocks to gain access to the upper portion of a retaining wall. The makeshift stairway did not contain any railings of any kind.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:    a) The employer failed to provided safe access to employees at a commercial construction site located at1500 W College St, Springfield, MO 65806, when employees were observed carrying large sections of metal roofing materials up a ladder that substantially increased the potential for lose of balance resulting in a fall.
