29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section:    a)  At the jobsite, on or about 4/27/17:  A worker was using a hand held grinder to cut masonry veneer without a guard on the grinder.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a)  At the jobsite, on or about 4/27/17:  An employee was working from a scaffold platform approximately 13 feet above ground and where that platform was not fully planked between the uprights.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  At the jobsite, on or about 4/27/17:  An employee was accessing a blue tubular welded frame (narrow format) scaffold platform approximately 13 feet above ground without fall protection of any kind.    b)  At the jobsite, on or about 4/27/17:  An employee was working from a blue tubular welded frame (narrow format) scaffold platform on brackets approximately 13 feet above ground without fall protection on the open end.      c)  At the jobsite, on or about 4/27/17:  An employee was working from a tubular welded frame (6' walk thru) scaffold platform where there was no mid-rail.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  At the jobsite, on or about 4/27/17:  An employee was applying grout to the fascia of the building, from an adjacent 10:12 pitch dormer roof without fall protection.  The employee worked from chicken boards and a roof brow.
