29 CFR 1910.29(a)(2): (2) Provide and install all fall protection systems and falling object protection this subpart requires, and comply with the other requirements in this subpart before any employee begins work that necessitates fall or falling object protection: (a) On or about May 1, 2017, an employee working out of a basket on a Altec Insulated Aerial Device was not provided with appropriate fall protection, in that the six foot lanyard provided by the employer exposed the employee to being flung from the basket when the basket fell 15 feet. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person. (a) On or about May 1, 2017, the Altec Insulated Aerial Device was not set up by a competent person, in that the lift was not properly stabilized, the outriggers were not extended in front of the vehicle and only one stabilizer was in place, thereby exposing an employee to a 15 foot fall. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.453(b)(2)(vii): The brakes shall be set and when outriggers are used, they shall be positioned on pads or a solid surface. Wheel chocks shall be installed before using an aerial lift on an incline, provided they can be safely installed. (a) On or about May 1, 2017, the Altec Insulated Aerial Device an employee was using was not properly stabilized, in that the outriggers were not extended in front of the vehicle and only one stabilizer was in place, thereby exposing the employee to a 15 foot fall.
