29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): Second floor, Wing B - Employees were performing construction activities and exposed to fall hazards of 12 feet or more in the following instances and were not protected from falling: a. Guardrails were missing or not at the proper height for several window openings on the second floor; b. A mid-rail was missing from the elevator shaft opening; c. The atrium opening was missing a guardrail in one area and mid-rails in three other areas. d. Employees were performing roofing operations with no fall protection.
