29 CFR 1910.22(c): (c) Access and egress. The employer must provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces. On May 9, 2017, at the Terminal, Door #30, a dock worker was exposed to a fall hazard due to the lack of a safe means of access while climbing onto a crate to stabilize the load being loaded up on top of a skid, approximately five feet off the ground.

(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: On May 9, 2017, at Terminal, Door #30, a dock worker was exposed to a 5 ft. fall hazard while laying on a crate to stabilize the load so that it would loaded on a skid and the crate flipped over and the worker fell to the trailer floor.

29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On May 9, 2017 at Terminal, Door #30, a sit-down forklift operator was moving a long, unstable crate on the tines of the forklift without any means to stabilize the load before entering the tractor trailer when his helper tried to stabilize load by laying his body on the crate and the crate flipped over and caused the helper to fall to the ground, injuring his shoulder and arm.
