Scot Nowak Construction, LLC project site located at W3926 Hwy 32, Ada, WI: On and about May 15, 2017, employees were exposed to falls while using a Genie boom lift when they had not been trained to recognize and avoid hazardous conditions or activities or to understand and follow the OSHA standards that apply to the use of aerial lifts or to understand and follow the manufacturer's cautions, warnings or prohibitions contained in the operator's manual.  Employees were not wearing personal fall arrest systems, were climbing on and over the guardrails, and using the boom lift as a material hoist by using unauthorized brackets connected to the platform of the lift.
