29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On 05/16/2017, C & J Seamless Guttering was performing work from a roof of a commercial structure located at 475 Mirlargo Shore Drive, Cottleville, MO.  The exposed employee was not utilizing any type of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On 05/16/2017, C & J Seamless Guttering was performing work from a roof of a commercial structure located at 475 Mirlargo Shore Drive, Cottleville, MO, and the access ladder was improperly positioned.  The ladder did not extend at least three feet above the upper landing or edge of the roof.
